If you require any vaccinations relating to foreign travel you will need to complete our travel form. This can be done on-line by clicking the link below, or you can come to the surgery and collect a hard copy. Details needed will include which countries and areas within these countries which you are visiting, to determine what vaccinations are required.
When you have submitted the form, you will need to telephone the surgery a few days later to find out if you require any vaccinations. If so make an appointment with the Practice Nurse.
This should be done at least 6 weeks before the intended travel date to allow time for administration and for adequate immunity to develop. Routine vaccines are free of charge but some require a private prescription which will incur a fee. This is because travel vaccination is not included in the services provided by the NHS and therefore can be a chargeable service.
