Applying to Register with the Practice

Registration Form

Glover Street Medical centre is home to two Practices, Kings Practice and Victoria Practice.

The Practice shares the registration of new patients with Victoria Practice here at Glover Street Medical Centre. Consequently there will be times when each practice will not accept new registration applications. Please be aware we do not routinely accept registration applications from patients already registered at an existing Perth City practice.

To register you will need to fill out a Registration form and Medical Questionnaire for each member of the family. When registering you will be asked to provide 2 proofs of identity, one of which should show your address, and one photographic.

If you have complex medical problems and/or take multiple medications  you will be asked to make a New Patient Appointment with the Doctor or Practice Nurse. Should you fail to keep this Registration Appointment without letting us know, you may not be accepted onto the Practice list.

You may fill out a registration form on-line, print it and bring it in with you on your first visit to the practice, alternatively you can print a blank form, fill it out manually and bring it in with you on your first visit to the practice.  We will need a hard copy for your signature.
